The training meetings for this mission trip are critical as well as mandatory. The meetings, which may be virtual or in person, are designed to ensure success of the mission. The meetings will begin in September of 2021. In addition, each team member will be required to attend (3) fundraisers from September 2021 to May of 2022. Funds raised will be used exclusively for food donations in Peru. Failure to attend scheduled events will disqualify the applicant from the trip. Please sign below your agreement to commit faithfully to meetings and fundraising events.

 

If you are selected, we will require a $100 non-refundable application fee to hold the date.  The application fee will be deducted from your final balance.

Medical & Liability Release

Permission & Liability Release

 I am aware that the mission trip to Sabandia, Arequipa, Peru poses risks included but not limited to : sickness, crime, political instability, personal injury, death, as well as similar or dissimilar risks. I am voluntarily participating in the mission trip with the knowledge of the risks involved. I hereby agree to accept any and all risks involved. I hereby agree to accept any and all risk of injury that may result from my participation in the mission trip. I also agree to not to sue or attach any assets for and from all claims of any nature of Sembradores de Semillas Mission Trips officers, directors, or employees. I also understand that I am giving up liability as a result of known or unknown, including but not limited to all liability, on account of death, injury, or damage resulting from negligence or other acts, however caused, as a result of my participation in the mission trip.

     Further, authorization and permission is hereby given to Sembradores de Semillas to furnish necessary transportation, food, and lodging for this participant.

     The undersigned further agrees to hold harmless and indemnify Sembradores de Semillas for any liability sustained by said organization as the result of the negligent, willful, or intentional acts of said participant, including expenses incurred attendant thereto.

     If during the course of the mission trip the participant should become ill or sustain any injury requiring medical attention, I hereby authorize Thomas Blaszczyk, Maria del Carpio, or the listed chaperones _____________________________ to obtain emergency medical services on my behalf. I will assume financial responsibility for the bills incurred.

     I have carefully read this agreement and fully understand its content. I am aware that this is a release of liability and a contract between myself on the one hand, and Sembradores de Semillas Mission Trips or their affiliates on the other hand. No oral representations, statements, or inducements apart from the agreement have been made to me. I sign this agreement of my own free will.

Photo/Video Release

By signing this application form, I hereby grant permission to Sembradores de Semillas Mission Trips to the rights, without payment or any other consideration, of my image, likeness, and the sound of my voice as recorded on audio or video tape. Photographic, audio, or video recordings, may be used by Sembradores de Semillas Mission Trips for the following purposes: informational presentations, promotional materials, newsletters, website, and social media venues.

Notice of Understanding

  • Completion of application does not guarantee a place on the mission trip

  • Each application will be reviewed by staff

  • In person interviews will be part of the application process

Financial Notice of Understanding

  • It is my responsibility to secure necessary finances for the mission trip

  • Trip balance must be paid 60 days before scheduled trip

  • Cancellation policy : Application fee of $150 is non-refundable. Application fee is due once the applicant has been accepted.

  • If cancelled 30 days or more before the trip – 50% refund

  • If cancelled 21 -29 before the trip – 40% refund

  • If cancelled 14 – 20 days before the trip – 30% refund

  • If cancelled 0 – 19 days before the trip – 0% refund 

  • Any funds that have been raised by the attendee through fundraising or mission events will not be returned in cash. They may be used for another trip or designated for mission use. The funds will have 1 year to be used. If not, they are forfeited to the mission.  

  • Extenuating circumstances ( illness or the death of a family member ) for trip cancellation will be reviewed on a case by case basis

  • Refunds for airfare are determined by LATAM Airlines - please refer to ticket

  • I understand that the team leader reserves the right to ask me to return home if my behavior is destructive to the team, the ministry, or the host community. The following is an immediate return home:

  • Alcohol use, drug use, or violation of Peruvian laws. Any additional cost incurred, as a result of this action, will be my responsibility. Sembradores de Semillas Mission Trips is not responsible to reimburse my trip expenses.   

Authorization

  • The information on this application form and addition forms is correct to the best of my knowledge. Additionally, I have also read and agree to abide by the policies set forth on all forms, and I support the mission of faith as supported in the supporting statements of Sembradores de Semillas Mission Trips.
